UPGRADE REQUEST FORM

Customers with an Out-of-Warranty Insulin Pump:

If you are currently out-of-warranty and interested in upgrading to a new model, please submit this form and you will receive a call to discuss your upgrade options.

• Medtronic will verify your insurance and provide an estimate of out-of-pocket costs
• Medtronic offers trade-in credits, payment plans, and financial assistance based on eligibility
• A prescription will be required to receive your new product. Please consult with your healthcare provider
  • Additional documentation and/or authorization may be required based on your insurance
  • Note: Individual products have different labeling, please speak with your healthcare provider to discuss the preferred product for your therapy needs.
• Training and onboarding support will be provided by Medtronic

Customers with an In-Warranty CGM transmitter:

• Depending on your eligibility you may be able to exchange your transmitter for $99 after trade-in credit. Terms and conditions apply
• If eligible, your remaining warranty will carry over
• Medtronic will assist with placing a new order of sensors compatible with your new transmitter

Customers with an Out-of-Warranty CGM transmitter:

• You may purchase a new CGM transmitter and sensors through insurance.
